- The Data Entry Form Templates window displays. For more information on adding a Data Entry Form, see Add a Data Entry Form.
- Click the row in the Data Entry Form Templates window corresponding to the template that you would like to use.
- Click the OK button.
- The Data Entry Form Editor window displays the selected template.
- Enter the Data Entry Form data. For information on using the Data Entry Form Editor window, refer to Display the Data Entry Form Templates Window and Filter Templates.

Adding a Data Entry Form from within the Location Summary window automatically assigns the location to the Data Entry Form. Adding a Data Entry Form from within the Land Summary, Activity Summary, Accident Summary, Victim Summary, Education Summary, or QM Summary windows automatically assigns the location of the item to the new Data Entry Form. |
After the Data Entry Form is approved, you must close and reopen the summary window for the Data Entry Form count to increase, and for the Data Entry Form to display in the Data Entry Forms window.
