To create a Data Entry Form template:

  1. From the Customisation menu, select Data Entry Forms.
  2. From the Data Entry Forms menu, select Design Templates.
  3. The Data Entry Form Template Designer window displays.
  4. Refer to the following sections for the steps to add form elements to and remove form elements from your Data Entry Form template:

Default Templates

IMSMA includes the following default Data Entry Form templates:

  • Accident and Victim
  • Land
  • Activity
  • Location
  • Education
  • QM

The default Data Entry Form templates are listed in the Default Templates folder which is located within the Template Folder.
