To add a Data Entry Form from the Workbench:

  1. Select one or more Data Entry Forms and click the FieldTemplateIcon.png button.

  2. The Data Entry Form Templates window displays.
  3. Select the appropriate template.
  4. Click on the OK button or double-click on the selected row.
  5. The template opens for Data Entry.
